1.What about my appointment?

Changing appointments requires 48 hours notice to avoid a charge
Late arrival: if you arrive 15 min. late, or fail to show, a charge will apply
Each patient receives individual attention and late arrivals means delays for subsequent patients. We do our best to be on time for your appointment.
Please inform us if you have a conference date scheduled with your orthodontist.
You must bring your Prescription Card to the appoinment.

2.What is this for and why do this?

Your orthodontist needs a complete record of your teeth, jaws and head to plan your treatment.
A.D.I has specialized X-ray and photo equipment for your diagnosis.
We fill the prescription as completed by your orthodontist. Any questions concerning the amount or need for the x-rays, is best directed to your orthodontist.

Please remove: all metal hairpieces, all earrings, studs, and piercing from the neck up and all removable appliances or dentures.
Photographs are taken of the face and teeth for most doctors. You may want to bring a brush or comb.
Disposable toothbrushes are available if you are unable to brush before arriving.
Please Note: parents may accompany the patient but must be out of the x-ray room when x-ray is taken; friends and siblings must wait in the reception area.
Please wait until x-rays and photos are reviewed before leaving the office. This will prevent return visits for retakes.
Your presribed survey will be delivered to your orthodontist in approximately 2 weeks.

We are not dental providers for insurance plans.
On request we can print a generic insurance form from our system that you can then complete and submit for re-imbursement of eligible benefits.
Payment in full is expected at the time of appointment; we accept Mastercard, Visa, checks and cash.
